The Doctors welcome the registration of new patients who live within our practice area.
The practice covers the whole of the Keighley area extending as far as Crossflatts, Eastburn and Crossroads. Please use this catchment area map to see if you live within our Practice area.
If you wish to register with the practice, please complete this online form
If you do not wish to use our online registration form you can come to the surgery reception between 12:00pm and 3:00pm Tuesdays, Wednesdays or Thursdays where you will be asked to fill in some forms including a health questionnaire.
If you know your NHS number please bring this with you as this can help the registration process. If you don't know it please don't worry, we can trace it for you.
